How to report on form custom section fields in Story report?

Resolution
The design is different between Single Template and All Templates when locating the section custom fields.
Single template needs to join table "xxx form section custom field" in order to select the custom fields. The actual custom field name will show in the list after configuring the custom element in form template.
Sample single PM Template:
In All Templates, it will show "Section Custom Field".
