3296204 - As the IAS administrator, how to deal with the situation that end user was asked for the passcode, but the passcode is unknown or incorrect

When end user tried to login application via IAS, after the logon(username/password, or other credential/IDP), passcode is asked by IAS.
The end user cannot login due to the following cases.
- The the passcode device is unknown.
- Error "Wrong passcode; enter the passcode again"
