3296204 - As the IAS administrator, how to deal with the situation that IAS is asking end user for passcode, but the passcode is unknown or incorrect

When end user tried to login application via IAS, in IAS login page, after inputting username and passcode, passcode is asked.
The end user cannot login due to the following cases.
- The the passcode device is unknown.
- Error "Wrong passcode; enter the passcode again"
