3354143 - Mitigation Issue in Access Request after upgrade - Request cannot be approved and mitigate XXX Risks

Currently our development version is upgraded to V12.0. Post upgrade while testing, facing a mitigation issue while processing the request.

Access Request is submitted with the risk role and the approver performed run the risk analysis to mitigate the risk ("Approve despite risk" is unchecked in MSMP stage config). Once all the risk are mitigated successfully,  re-run the risk analysis and the output shows "No Violation". After this when the approver hit the Submit button, it throws an error - "Request cannot be approved and mitigate XXX Risks", wherein in access request there is no open risk to mitigate now.

Tried setting the parameter 1074 as both YES & NO and the mitigation type for all the risk was saved in Temporary and Permanent table successfully.
