Symptom
- User already submitted a survey when completed an online item. Is it possible to trigger the survey again for the same item?
- Users see 2 surveys on the Learning Plan for the same blended item (Instructor-led with Online Content).

Resolution
For Online item, it is not possible to trigger the survey again unless user completed the item again or got a new Learning History for that item.
For Blended item, there are 2 way to trigger course feedback survey.
One way is that a survey will be automatically assigned after user completed the online content or admin recorded Learning History manually.
The other way is that admin can initiate a survey via Class > Action menu > Initiate Survey.
If admin has initiated a survey for a Class, and later user get Learning History recorded with this particular Class ID, the survey won't be triggered again.
However, if admin has initiated a survey for a Class, and later user completed online content or get Learning History recorded without the particular Class ID, the survey will be triggered again. It is considered as the feedback for the Class part and the Online Content part.
User may see duplicate Surveys on their Learning Plan and both show as for the same blended item but actually one is for the Class and the other is for the Item.
On the survey for class, user can see Item description, Class Description, Instructor name and Primary Location fields on the survey header.
While on the survey for item, there is no only Item description field on Survey header.
