3397102 - "DBSQL_DUPLICATE_KEY_ERROR" Error while processing Dimension - BPC

Symptom

The user is getting a dump error when creating a new node in any BW hierarchy using BPC interface. It doesn't refresh hierarchy active version (A) and stay in modified version (M).
