3420019 - SAP IBP Login Error: "An unforeseen error occurred on the server."

Symptom

The following error message is displayed when logging in via SAP IBP, add-in for Microsoft Excel:

An unforeseen error occurred on the server.
If the error persists, contact your administrator with a detailed description of the steps that led to the error.
Also include the information found in the log (in the "About" group of the SAP IBP ribbon, select "Help", then "Log..." and copy the entire text shown in the text box).
