3465769 - unique constraint violated / duplicate key found in HANA database

Symptom

During an SAP application upgrade, a consistency check or an import of a migration, you get an error on the HANA side stating, for example

'unique constraint violated'
cannot CREATE UNIQUE INDEX; duplicate key found

Several documents with the same ID exist in the index
