3465808 - Service Instance not found during MTAR package deployment

Symptom

Deployment of MTAR package fails with:

Creating application "db" from MTA module "db"...
Binding service instance "hdi_db" to application "db"...
Error while checking service binding operations between app: "db" and service instance "ExampleServiceInstance-hdidb-example": Controller operation failed: 404 Not Found: Service instance ExampleServiceInstance-hdidb-example not found.
