3474616 - Details in Charm Notification Email Incorrect

An automatic notification email is triggered in charm which uses mail form attributes to provide some detail from the document, like status, title, or URL to the document. But on checking the contents of the email one or more of these details are incorrect.

For example, the URL given in the email doesn't link to the document it was triggered from.
