How to add extension fields to form template of SAP Sales and Service Cloud V2 system?

You can use the Adobe LiveCycle Designer (ALD) to add the extension fields. Please check this document in order to download ALD software: https://me.sap.com/notes/2187332.

1. Click on the User menu in SAP Sales and Service Cloud v2 system.
2. Go to Settings.
3. Open Maintain Form Template view.
4. Download the Form template you want.
5. Open .xdp file (template) in Adobe LiveCycle Designer and go to Data view.
   
   
6. Right click on "Dataconnection" and select "Connection properties".
7. Click Next.
8. In this screen, browse and select the .xsd file from the .zip you downloaded.
9. Click on Finish button.
10. In Data view tab, scroll down to Extensions.
    
    
11. Double-click the extension field name you would like to add to the form.
12. You will see the extension field is now seen in Design view, for example, in this case the extension field name is Testing23.
    
    
    
    
13. Under Design view, give a right click in the newly added extension field.
14. Click on Palettes and select Object.
15. On the right side, an object section will appear. Navigate to Binding tab in this section.
16. Click on Select data binding button.
    
    
17. Select "Use DataConnection".
18. Scroll down and hoover the mouse in extensions folder.
19. A list of extension fields will appear. Select the one with the name of affected field. (In case the extension field binding is not there, please save your file first and re-open it.)
20. At top-left, click on File and select Save.
21. In the system, go back to Maintain Form Template view.
22. Edit the affected form template and upload the new .xdp file.
23. Click on save & publish.
