Symptom
New hires are unable to submit the required Illinois State Withholding Form (W-4) in Onboarding. The form appears with all fields empty, preventing candidates from completing the submission.

Reproducing the Issue
- Log as a new hire.
- Click on "Provide additional data" tile.
- Click on the Illinois state W-4 form.
- All fields are blank and cannot be edited.
Cause
The issue occurs because the address3 field, which is used by the Illinois State Withholding Form, is not configured in the corporate data model. This results in the system being unable to fetch or populate the required field when the form is generated.
The system logs show an error indicating that the form is attempting to use an invalid or missing property: Invalid property names: FOCorporateAddressDEFLT/address3
Resolution
In order to confirm whether the address3 field is not configured, please follow the flow:
Please verify the property name by navigating to Admin Center > OData API Data Dictionary (filter: FOCorporateAddressDEFLT and notice if address3 is present) or by checking the entity metadata. Additionally, perform a Refresh Metadata to ensure that the system cache is up to date and not corrupted.
To resolve this issue, the address3 field must be correctly mapped in the system. You can choose one of the following options:

Configure the Address3 Field in the Corporate Data Model:
Add the address3 field to your corporate data model via Provisioning. This will ensure the Illinois State Withholding Form can retrieve the necessary data. -
Map Address3 to Another Field:
Alternatively, you can map the address3 field to an existing field in Employee Central (EC). This could be a field that is not currently in use or a similar field like address2.
Either of these actions will allow the form to populate correctly and resolve the issue of empty fields.
