3571487 - Compliance form fields are blank for candidate with 404 error in Browser Developer Tool - Onboarding

Symptom

  • Several fields are blank in compliance forms e.g. First/Last Name, SSN, Address, etc.
  • Example affected compliance forms:
    • Georgia State Withholding Form G-4
    • Voluntary Self-Identification of Veteran Status
    • Voluntary Self-Identification of Disability
  • If check Browser Developer Tool, see "404 page not found" error in Console/Network.
