3572937 - "Back to Author" Action Completes but Work Item is Not Reassigned to Initiator

• When selecting "Back to Author" in My Inbox, the UI confirms that the request has been sent back to the initiator.
• The work item is marked as "Completed", however, the request does not appear in the original initiator’s My Inbox.
• In SWIA, the Work Item Text is blank, which is not expected.
• In the Workflow Log:
  • The step for "Back to Author" may be untitled or blank.
  • The expected "Workflow Exit is activated" action is missing.
  • Only a dialog work item creation is recorded instead of the correct transition
