3586194 - What can be the root cause for duplicate entries in table CKMLHD?

Symptom

Table CKMLHD has duplicate entries existing which causes ITAB_DUPLICATE_KEY runtime error. 

The issue is usually seen when material master data is sent via IDocs and also with MDG involvement. The root cause is not a straightforward find.
