3592362 - SQL code 289: SQL message: cannot use duplicate index name

There is a object that has runtime and database object inconsistency in SE11. In SE14, there is no table conversion is processing but is shows a change recently. 

From the activation log and object log, it says: 

Table XXX could not be activated
(E- Table XXX (Error adjusting the database) )

SQL Exception: SQL code: 289; SQL message: cannot use duplicate index name: XXX~0 or cons
Statements for Table XXX could not be executed. 

If the error happened in table conversion step,  further investigation is needed.
