3621892 - Duplicate line items displayed in My Purchase Requisitions - New app after approval

Symptom

In the My Purchase Requisitions - New app, it is observed that after a Purchase Requisition (PR) item is created, ordered, and approved by an approver, reopening the approved PR results in the same line item being displayed multiple times. For example, item 00010 may appear duplicated several times in the item list.
