Symptom

During an SAP installation or upgrade using Software Update Manager (SUM), the process fails in the following phase:
PREP_INSTALL/SUBMOD_PREP_INSTALL/SHDINST_DB_PREP

Error messages observed in logs:
BR0301E SQL error -12560 at location db_connect-3, SQL statement:
'CONNECT SAPDR5SHD/****************'
ORA-12560: TNS:protocol adapter error
BR0310E Connect to database instance DR5 failed
