3684873 - Bank Account Management Email Notifications are not generated

Symptom

  1. When one approve or reject a bank account creation/modification no email notification is generated for the requester.
  2. The email notifications for the "Review Bank Accounts" process are not being triggered correctly. Specifically, the task link is not appearing in the emails sent from the system using the email template FCLM_BAM_INI_REVIEW_EMAIL.  The issue affects the customer's ability to navigate directly to the "Review Bank Accounts" application from the email notification. 
  3. SOST email notifications are being triggered to their personal email ID instead of the business email ID. During the approval of BAM requests via the Manage Bank Accounts app, emails are triggered from the approver's email address instead of a centralized sender email address.
