3698418 - ST12 shows time is spent on DB - database or application related issue?

Symptom

For analyzing the bad performance, an ST12 trace is generated while reproducing the issue. That shows the DB as top consumer, similar to below example:

ABAP trace:

ABAP        |     9.760.089    =    5,9 %  
Database  |  156.093.730    =    94,1 %  
System     |          64.494    =    0,0 %  
Total         |  165.918.313    =    100,0 %
