The proof of good health Contribution is displaying the same contribution value before and after approval.

1. Open a test user's profile.
2. Navigate to the annual benefits enrollment
3. Select a voluntary insurance plan configured with Proof of Good Health (Evidence of Insurability)
4. Choose a coverage amount above the non-EOI threshold (for example, above the limit that requires proof)
5. Observe that the contribution shown before proof and the contribution after approval are the same

The rate chart Factor was configured as -1, causing the system to take the employee contribution directly from the rate chart and preventing calculation of different rates for before- and after-proof scenarios.

1. Edit the plan's rate chart and change the Factor from -1 to a positive value (for example, 1, 10, or 100) appropriate for the plan’s calculation
2. Review and, if needed, update any related rate chart business rules so that contributions are calculated based on coverage and Proof of Good Health status
