3755649 - HTTP 400 Bad Request "Routing error for annotation file" for Fiori application due to duplicate entries in /IWFND/I_MED_VAA

Symptom

When opening a Fiori application in SAP Fiori Launchpad, the application fails to load.

In network trace (F12 Developer Tools), the metadata or annotation call fails with:

HTTP 400 Bad Request
Routing error for annotation file "<ANNO_TECH_NAME>" version "0001" based on service "<SRV_IDENTIFIER>"

Example from the reported case:

Routing error for annotation file "FINCS_ADDL_MD_MAINTENAN_ANNO_MDL" version "0001" based on service "ZFINCS_ADDL_MD_MAINTENANCE_SRV_0001"
