• User can open the To-dos tab on a factsheet and use “ask a question” but cannot save the item.
• On save, the error “could not create question” appears.
• Issue occurs regardless of selected assignee.
• Issue occurs across different fact sheets (not specific to one fact sheet).
• Admin users can perform the same action without error.
