Unable to remove ratings and comments option while using get feedback feature.

1. Open a Performance Management form.
2. Use the Get Feedback feature.
3. Observe that options for ratings and comments are displayed and cannot be hidden.

The Performance Management form template setting controlling Get Feedback behavior was not configured to hide ratings; by default, ratings and comments are available.

1. Open the Performance Management form template in the administration area.
2. check the option "Disable Ask for feedback edit routing."

3.Save the template changes.

4.Review an existing live form to confirm the change (no need to relaunch a new form).

5.Validate that the Get Feedback dialog now shows only the intended options.
