3779921 - "Request assessment service" button disabled in Quality Gates with message "User information is incomplete for creating service request' - SAP Cloud ALM

After answering all questions and accepting the follow-up assessment service from SAP in the popup in Quality Gate, the "Request SAP Assessment Service" button is disabled and shows "User information (first name, last name, email) is incomplete for creating service request".
