This document explains the available WTPA forms and the backend system logic used to determine how forms are assigned based on employee types.

SAP SuccessFactors Onboarding

NY WTPA list of the forms which are used in Onboarding:

• Notice and Acknowledgement of Wage Rate(s) for Temporary Help Firms (LS 51)
• Pay Notice for Hourly Rate Employees (LS 54)
• Pay Notice for Multiple Hourly Rates (LS 55)
• Pay Notice for Employees Paid a Weekly Rate or Salary for a Fixed Number of Hours (40 or fewer in a week) (LS 56)
• Pay Notice for Employees Paid a Salary for Varying Hours, Day Rate, Piece Rate, Flat Rate or Other Non-Hourly Pay (LS 57)
• Pay Notice for Prevailing Rate and Other Jobs (LS 58)
• Pay Notice for Exempt Employees (LS 59)

Type of the form which will be generated for a candidate is determined according to the following logic:

1. When you activate the WTPA feature in Super Admin site you must select do you want to use LS51 or LS54-LS59.
2. If you select to use LS51 --> LS 51 form will be generated
3. If you select LS54-LS59, the keys "JobPayType" and "FLSAStatus" are used for calculation
     if "FLSAStatus" = "E" --> LS 59 form will be generated
     if "FLSAStatus" = "NE" and "JobPayType" = "HOURLY" --> LS 54 form will be generated
     if "FLSAStatus" = "NE" and "JobPayType" = "MULTIPLE HOURLY RATE" --> LS 55 form will be generated
     if "FLSAStatus" = "NE" and "JobPayType" = "SALARY FOR FIXED # OF HOURS" --> LS 56 form will be generated
     if "FLSAStatus" = "NE" and "JobPayType" = "VARIABLE SALARY" --> LS 57 form will be generated
     if "FLSAStatus" = "NE" and "JobPayType" = "PREVAILING RATE WORK" --> LS 58 form will be generated
     if "FLSAStatus" = "NE" and "JobPayType" = "SALARY" --> LS 59 form will be generated
