Customers may observe one or more of the following:

• The Issue Owner cannot enter review comments.
• Review/validation does not occur as expected.
• "Reviewed By" and "Reviewed On" are empty.
• The Remediation Plan is completed but the expected validation step is missing.
• The customer expects a separate approval after remediation.
