3808559 - Surveys not displayed or addable in Visit after DTT migration; "Survey {0} not found" error

• After transferring Visits via Data Transfer Tool, surveys are not displayed within the Visit although a completed survey exists and the Visit header indicates one is available.
• Error on opening a Visit: "Survey {0} not found." 
• The Objectives tab does not list any survey despite one being shown in the General section.
