3813000 - Why is User Profile Changes Version 2 report showing activities that do not match with my actions?

Symptom

I opened terminated user profile, removed user as a member from groups in the User Group Membership field and saved profile.
I opened the report User Profile Changes Version 2 and found in column ChangedBy values under my name.
These values are not matching with my actions. I did not add home country or work country into user's profile.
