3815786 - Why is a trip approved remark in my PNR when the approver rejected the trip?

An online booking was made that required approval. The Approver rejected the trip. There are now two remarks in the reservation. One remark begins with the CB/ indicator showing Concur added the Rejected information (CB/APP/REJECTED) and the other remark is a General Remark showing the trip was approved. If the Approver rejected the trip, where did the General Remark come from showing that the trip was approved?
