3816442 - Why do we have cash advances that are not issued by a Cash Advance Administrator?

I am the Credit Card Administrator for my company and I am looking at the Cash Advance Admin - all cash advances and have noticed that there are quite a lot of transactions, which have not been requested to my knowledge and whereby I have not physically provided currency and they are shown as being issued.
I am very concerned, who issued them?
