3819496 - Why are new cards named "Name_on_Card" and where are the transactions?

Symptom

We have cards named "Name_On_Card", and the last segment matched an existing record. Credit card transactions are not feeding through.
Or
 We have received an email to assign a new AMEX Business card but the name field is blank. It shows- "Name_on_Card" as the employee name. How do I know whose card this is?
