3819540 - Error: "The file was not uploaded successfully. See your System Administrator for more information."

Symptom

I have tried to upload employee import file via Import/extract monitor however, I am getting the same error numerous times with a message: "The file was not uploaded successfully. See your System Administrator for more information."
