3826612 - User Import Error: "The combination of Employee ID and Login ID or Employee ID and Employee Last Name must be unique."

Symptom

I receive the following field validation error when trying to upload a User Import file:

"X user records failed field validation. These user records appear in the table below and will not be processed."

"The combination of Employee ID and Login ID or Employee ID and Employee Last Name must be unique."
