3826734 - Why do I get Travel Policy Violation: "Airfare is Greater than the Least Cost Logical Airfare" When the Lowest Price Fare has been selected?

A traveler selects the lowest fare within policy, but receive a Lowest Fare Warning that the lowest fare was not selected however the information in the error shows the Estimated Selected Fare to be higher than the Quoted Fare and Estimated Least Cost Logical Fare offered if for the flight that has already been selected - why does this happen?
