• In the “Survey Answers with Attachments” data source (FNDQRERUNTATTACHB), the “Survey Response Attachment” field displays “Not assigned” for question-level attachments that exist and are viewable in the UI.
• Rows for generic visit task-level attachments appear in results even when the selection is fixed to a specific Survey ID; these rows show no Answer Text (“#”).
• Filtering on Question Description cannot exclude rows with “#” (both with an IN list and with “is not #” in Advanced selection).
• The data source does not expose suitable linkage fields to reliably join to Visit/Account/Employee context.
