• Login attempt fails with error: " Details: No additional information is available at this time."
• After the error occurs, the user cannot return to the login page until the browser cache is cleared.
• Clearing the cache and changing the password allows a single successful login; after logout, the same error recurs.
• Non-SSO login (default identity provider with username and password).
