• Attempting to add a custom banner in Learning Administration results in an upload failure.
• The interface displays the message "fail to upload" when selecting an image for the banner.

1. Go to Learning Administration.
2. Go to Banner.
3. Add a new custom banner.
4. Attempt to upload a compliant image file.
5. Observe that the upload fails with the message "Failed to upload file".

• Private/incognito browsing mode blocks required cookies, which prevents the banner image upload from completing.

1. Exit private/incognito browsing mode and use a standard browser session.
2. Ensure the browser allows cookies for the application domain.
3. Retry uploading the banner image.
4. If the issue persists, verify the banner image meets requirements (for example, 1920x250 pixels, .JPG or .PNG, and ≤ 2 MB), then try again.
